What is LTSS?
LTSS encompasses a range of assistance solutions for individuals who have functional limitations due to age, chronic illness, or disability. LTSS is typically provided on an ongoing and often daily basis, with the goal of supporting individuals' independence, inclusion, and access to needed care and resources. With a whole-person, person-centered approach, LTSS promotes dignity and independence by addressing the broader needs of individuals.

A Dedicated Care Manager
Provided to everyone enrolled in LTSS, a care manager acts as members’ personal guide. They help manage and create care plans, and they're also a go-to resource for any questions a member might have about their benefits.
Family and Caregiver Support
We provide emotional support and valuable resources for member families and caregivers, recognizing their vital role in our members' care journey.
Supported Employment
A dedicated job support program that helps members find employment and overcome job-related obstacles. This includes added ride coverage, GED preparation, and personalized career advice.
